Reimbursement deadline for Health Benefit Card purchases in 2014 approaching

The deadline to apply for reimbursements for medical expenses paid out of pocket in 2014 with the Osage Nation Health Benefit Card is March 28.

Reimbursement forms, enrollment forms, re-enrollment forms, list of eligible health expenses, directions on how to access online data and set up profiles, can be found online at https://www.osagenation-nsn.gov/member-services/health-benefits-information.

Osage Nation tribal members 18 years and over as of Dec. 31, 2014, with a valid membership card (this is not a CDIB), are eligible for the $500 Health Benefit Card. Tribal members over the age of 65 years old as of Dec. 31, 2014 are eligible for $1,000.

Unspent money from the previous calendar year, for tribal members re-enrolling for the health benefit, will roll over to the 2015 calendar year as soon as re-enrollment forms are turned in and processed.

Mutual Assurance Administrators (MAA) is still administering the Health Benefit Card, despite the company being bought by HealthSmart in November of 2013.

Due to 2011 Federal IRS changes over-the-counter eligible purchases will only be paid for by reimbursement.
